NON-ROSTERED GUEST PLAYER INSTRUCTIONS

2013-2014

Instructions for Parent/Guardian of Player:

  1. Using the Non-Rostered Guest Player (NRGP) link at the bottom of this screen, access the on line registration system.
  2. After reviewing the material on the opening screen, click the button at screen bottom to open the Registration Form.
  3. When data entry is complete, please review the form to verify the information is correct (especially the date of birth and the answers to questions on International Clearance).
  4. After data review (and correction if needed), click the Submit Form button.
  5. Print the Guest Player Registration form and the Medical Release form
    • Fill in any missing information on the Registration form.  It must be signed and dated by the player and a parent or guardian.
    • Fill in any missing information on the Medical Release.  Before signing, check with the manager on whether the team requires it to be notarized.
  6. Click the button to pay the player registration fee on line by credit card.
  7. Print a copy of the payment receipt – this completes the on line process.
  8. A digital photo of the player must be sent to the team manager via e-mail.
    • Details on photo size, file format, and file size limit can be found on the MYSA web site.  From www.msysa.org, select Registration at the left; on the resulting screen, select the document “Digital Photo Guidelines.”
  9. The registration process for the NRGP is completed only once per seasonal year.  The US Youth Soccer card can be used at any number of sanctioned tournaments, but it must be turned in after each tournament. 
    • Within three days after the tournament, the original Registration Form and the US Youth Soccer card must be turned in to the MSYSA Registrar that processed the registration.

Instructions for Team Manager:

  1. Obtain from the player/parent:
    • Non-Rostered Guest Registration form (verify that it’s complete, and has been signed and dated by player and parent).
    • An acceptable proof of age (must meet same requirements as rostered players)
  2. Review the player photo sent to you by the family; confirm that it meets requirements in the Digital Photo Guidelines document.
  3. Forward the player photo by e-mail to your assigned MSYSA Registrar.  Be sure to include the player name and date of birth in your e-mail.
  4. Bring the following to an MSYSA registrar for approval:
    • Completed and signed Non-Rostered Guest Player Form (original plus one copy.
    • Acceptable proof of age.
    • Copy of credit card payment receipt.
    • Copy of the tournament’s approved Permission to Host form.
    • Completed Tournament Guest Player Form (original plus one copy; form is found on the Forms page on www.msysa.org).
  5. Within three days after the tournament, the original Registration Form and the US Youth Soccer card must be turned in to the MSYSA Registrar that processed the registration.
